Dr. Jennifer Schatz, a family physician in Ottawa, wanted to fulfill her CPSO quality requirements and reached out to OMD for advice. Katie Peter, an OMD Practice Enhancement Consultant, met with Dr. Schatz to share her experience and insight on the QI program focusing on potential activities and a practice improvement plan aligned with Dr. Schatz’s goals for her practice.  

CPSO’s QI Program helps Ontario physicians engage in self reflection, improvement, and meet their quality requirements. The program offers a flexible, collaborative and educational approach to QI, allowing physicians to earn Continuing Professional Development (CPD) credits.

Dr. Schatz decided she wanted to focus on improving her management of patients with diabetes. They discussed the need for more communication, on a consistent basis, for these patients. This was based on the repeated cycle of A1C testing required at the lab, followed by visits to discuss the results. Together, they defined an opportunity to optimize Dr. Schatz’s EMR to improve efficiencies and provide a better experience for these patients.  

Dr. Schatz and Katie first identified patients with diabetes so they could code them in the EMR and enable easier access to these patients’ charts.

They then defined each visit/appointment type. They developed email reminders for patients, so they would complete their required blood tests prior to their office visit. Patients with diabetes are ideally seen every three months, so at each appointment, they needed a lab requisition to continue the cycle of care. Dr. Schatz and her team ensured this would be done during the scheduled appointment.  

As for Dr. Schatz’s results?  

“Significant improvement in monitoring diabetic patients with more frequent A1C levels and a better, more efficient experience for my patients,” said Dr. Schatz. 

Dr. Schatz and Katie would meet virtually, as needed, during the project to review and refine the next steps and troubleshoot any issues.
